Preheat the oven to 375°F (190°C).
Prepare the Meatloaf Base:
In a large bowl, mix together the ground beef, chopped onion, minced garlic, breadcrumbs, milk, egg, Worcestershire sauce, salt, and pepper. Press the mixture evenly into the bottom of a greased baking dish.
Top with Ketchup:
Spread the ketchup evenly over the meatloaf layer.
Bake:
Place in the oven and bake for 25 minutes.
Make the Mashed Potatoes:
While the meatloaf bakes, boil the sliced potatoes until fork-tender, about 15 minutes. Drain and mash them with sour cream, milk, and butter. Season with salt and pepper to taste.
Assemble the Casserole:
Once the meatloaf has partially cooked, spread the mashed potatoes over the top. Sprinkle with shredded cheddar cheese and chopped green onions.
Finish Baking:
Return the dish to the oven and bake for an additional 20 minutes, or until the cheese is melted and bubbling.
Rest & Serve:
Let the casserole rest for 10 minutes before slicing and serving.
